These Terms of Use (“Terms”) govern your use of the Precision Wellness website (the “Site”), operated by 

Altisera Healthcare Management d/b/a Precision Wellness (“Precision Wellness,” “we,” “our,” or “us”). By 

accessing or using this Site, you agree to these Terms. 

Educational Purpose Only 

The Site and all related content, consultations, forms, and communications are provided solely for 

educational and informational purposes. Precision Wellness does not provide diagnosis, treatment, 

individualized medical advice, or clinical care through this Site. 

No Physician-Patient Relationship 

Use of this Site, submission of forms, participation in consultations, communications by email, phone, or text, or interactions with Precision Wellness do not create a physician-patient relationship. 

No Medical Advice 

Nothing on this Site constitutes medical advice, diagnosis, or treatment recommendations. Visitors should 

consult their own healthcare providers for individualized medical guidance. 

Consultation Services 

Precision Wellness offers paid educational wellness consultations. These consultations are 

informational only and are not clinical services. 

Consultation fee: $75 unless otherwise stated. 

Refund / Cancellation Policy 

Consultation fees are refundable if cancellation occurs at least 24 hours before the scheduled appointment. 

Late cancellations, missed appointments, or no-shows are non-refundable. 

Third-Party Services 

We may use third party providers including scheduling, payment processing, analytics, communication, and related wellness resources. Precision Wellness is not responsible for third-party platform operations, privacy practices, or content. 

Third-Party Wellness Information 

Precision Wellness may present educational information about third-party wellness products or services, including partner offerings. Such information is provided for educational purposes only. 

Intellectual Property 

All content, branding, graphics, text, logos, trademarks, and website materials are the property of 

Altisera Healthcare Management d/b/a Precision Wellness unless otherwise noted. Unauthorized 

reproduction is prohibited. 

No Emergency Services 

Do not use this Site for urgent or emergency medical needs. If you are experiencing a medical 

emergency, call 911 immediately. 

Limitation of Liability 

To the fullest extent permitted by law, Precision Wellness disclaims liability for damages arising from use of 

the Site, consultations, communications, or third-party resources. 

Arbitration Agreement 

Any dispute, claim, or controversy arising out of or relating to these Terms, the Site, 

consultations, communications, transactions, or related interactions with Precision Wellness shall 

be resolved exclusively through final and binding arbitration rather than in court, except where 

prohibited by law. 

Arbitration shall be administered by the American Arbitration Association (AAA) under its 

applicable consumer or commercial arbitration rules, as appropriate. 

Arbitration shall occur in the State of Texas unless otherwise mutually agreed. 

By using this Site, you knowingly waive the right to: 

∙a jury trial 

∙participate in a class action lawsuit 

∙participate in a class arbitration proceeding 

∙bring claims as a representative plaintiff 

Each party shall bring claims solely in an individual capacity. 

This arbitration provision survives termination of these Terms. 

Governing Law 

These Terms are governed by the laws of the State of Texas.
